If your horses have been registered at another veterinary practice, we will need to contact them for their full clinical history to ensure a smooth transition of veterinary care, so please provide their details as well. Please be aware that due to the General Data Protection Regulations, your previous practice will contact you to obtain your consent prior to sending this information.

Have your horse’s passport(s) to hand as we would like to record their passport and microchip numbers. All equines in the UK must have a passport. Those registered after 2009 must be permanently identified with a microchip. As from October 2020 all equines in the UK must be permanently identified with a microchip. If your horse was passported prior to 2009 and is not chipped currently, you will need to arrange an appointment for this to be performed.
